You need to be registered to be seen. Click on the link below for registration packet. Packets are also available at the school nurse, Jacqueline Nichols’ Office.
Return completed packets to: HVRHS, Attn: School Based Health Clinic, 246 Warren Turnpike Road, Falls Village, CT 06031
Completed packets can also be turned into the Assistant Principal’s Office or the Main Office at the high school.
